An employee was standing on a 14-inch, 60-foot-long, 73-pound-per-foot H-pile lying on dunnage. A rough-terrain forklift was being used to move a 6-inch, 12-foot-long channel on its forks. The end of the channel clipped the end of the H-pile, causing it to roll off the dunnage and strike the employee's lower leg. The employee suffered fractures to the lower leg and ankle, with no open wounds.

The stacker was then freed of the rail that was displacing it and the stacker fell onto the employee's foot resulting in multiple fractures as well as an open wound on the bottom of the foot that extends from heel to toe.

On May 29, 2015, an employee was working inside a sulpha treat reactor vessel using a high pressure (5000 PSI) water sprayer gun to cut off the congealed catalyst material inside. The employee slipped and fell; the high-pressure water sprayer hit the employee's right foot and punctured a hole through the rubber boot. The employee sustained a puncture open wound on the right foot, second toe. The employee was hospitalized and received treatment.
